This is Mega Charizard Yex (Mirror Version).
It is a card included in Starter Deck No. 001.
Immediately after opening the pack, I placed it in a card sleeve and an OPP bag, and have been storing it in a dark place.

The cut along the right edge of the card is rough and has white, jagged edges (see Images 2 and 3).
