Even the GEM MINT 10 items, which are PSA's highest rated, may have some initial scratches (white chips, holo chips, vertical and horizontal lines, etc.), minor scratches, and, although rare, scratches or foreign objects on the surface of the case from when the item was returned from the grading company. There may be scratches on the surface of the case or foreign objects on the surface of the case that have been there since it was returned from the appraisal company, although they are rare.
